I have been on lithium for about 4 months and can assure you, with careful awareness one can go to the Gym. My Psychiatrist has even OK'ed me to have a 10 minute Sauna after my Gym session. With lithium being a salt one must be careful not to sweat excessively as this could cause depletion of sodium in the body and may lead to lithium filling in the void.
As far as the first few days on Lithium, it is a bit strange but worth working through. I found that after 5 days my thinking - which has been fuzzy and dull - became much smoother and along with Amitriptyline my depression lifted.
The blood tests are taken frequently to begin with to ensure that one is within the theraputic window. Once this is settled then tests are every three months to check lithium levels and thyroid function. However if dosages or lithium brands are changed then this will neccessitate another blood test.
I hope this helps and you find as much relief through lithium as I have.
Regards
