My MD, who is usually pretty conservative, put me on 4mg/day after a severe GAD attack a couple of months ago. The pharmacist commented that it was a fairly heavy dose, and to be careful driving etc., but I find it's not that much for me - I have a high tolerance to benzos and just generally to most anxiolytics and sedatives, but from what I read here, 2-4 mg/day is not that uncommon.
I have seen somewhere recently (sorry can't recall ref., maybe rxlist.com) up to 20 mg. / day being used to treat seizures, and I've taken as little as 0.5 mg in the past, so the safe/useful range must be pretty wide.
My MD also says it is about the least likely of all the benzodiazepines to cause problems with tolerance/dependence, partly because it is fairly slow acting and has a long half-life, and maybe some other reason peculiar to its nature. There are people who have posted here & elsewhere who have been on substantial doses for years with no problems.
