I would add amitriptyline to this list even though I have seen this combination not contraindicated in another reference. Amitriptyline's main effect is its H1 histamine blockade, but it is still relatively serotonergic (selectivity ratio 1.6 to 1 in favor of NE reuptake blockade over serotonin reuptake blockade).
