Have you talked to your regular doctor about this? maybe you could see a specialist--physical therapy sounds like it might prove to be very helpful! maybe you just need to re-train the muscles? I'd also look into massage therapy--I had very tense jaw muscles last year (well, still do!), but was helped by a massage therapist.
