Officially..."When satisfactory response occurs, gradually reduce to maintenance level."

10mg in the morning...but I don't hear a lot about maintenance dose and MAOIs...everyone I seem to know, will take the med for months/years at the effective dose (30-40mg) and then gradually stop.

Well, in theory, you should get the same response to a maintenance dose as you would the effective dose (after a full response to the effective dose)
When it comes to MAOIs, a maintenance dose is possible only because of the amount of time your cells need to re-synthesize MAO. If you have a full response to 40mg at week 8 (depending on your Dr.) s/he may decrease to a maintenance dose of 10mg, which whould still keep MAO activity down (since, in theory, theraputic response occurs @ 80% MAO inhibtion)
In my opinion, after being on both Nardil and Parnate, a lot of the theraputic effects are immediate...for example, parnate's immediate effects may involve stimulation of catecholamine release, etc.
