Whatever it is that troubles your friend, it surely isn't serotonin syndrome. That is an acute, emergent (same root meaning as emergency) condition. It would have been harsh, and it would have happened two years ago.
I can't think of a link between the Effexor and your friend's feeling unwell all this time.
