Just an idea, but what if you wrote down the key points: the problems you're having, questions for the doctor, and possible solutions, if for example there are particular meds you're considering. Then take the list to the appointment with you.
You could explain to the doctor right at the beginning that you're very nervous, and really want to make sure you remember everything. I don't think a good doctor would find this weird, and any experienced doctor has dealt with many anxious patients. If you get really tongue-twisted, you might even show the doctor the list rather than reading it to him or her.
This could help take the pressure off you to have to verbalize everything when you're so anxious, and my guess is that the doctor would appreciate the effort you've made to clearly address your problems. This could also guide your doctor in the questions that he or she might ask you. I know a couple of people with severe anxiety who have used this approach, and their doctors responded very favorably.
