Sorry you're having a bad time. Here are a few thoughts that I hope might help.

Have you tried TCAs or MAOIs? I've never taken MAOIs but I have been taking imipramine for 20 years and it has continued to work, with the exception of a good many breakthrough episodes of major depression.

I'm taking zyprexa now myself. I think your concerns about long term use of zyprexa are valid. There is the possibility of movement disorders, though zyprexa is, iirc, least likely to do this. So, I think you have plenty of time to find the right solution, before you need really worry.

I don't think it's unreasonable to be considering ECT. I've never had it done, but my understanding is that it is inconvenient but quite safe and measurably more effective that ADs. I don't know if ECT helps SSRIs to work again.
