How to best quit Remeron?

I know some of you have certain ways you taper down meds to reduce the horrible discontinuation effects. Can anyone give me an idea how to best quit 30 mgs. of Remeron? I have been on it for a year. I am suspecting it is the med that has made me gain an extra 25 pounds.
