Dr. Ghaemi is extreme? In what sense? I know he doesn't subscribe to the views of Dr. Akiskal. It's Dr. Akiskal's opinion that a diagnosis of unipolar depression cannot be made until bipolar disorder has been ruled out. (That, and his endless subcategorization and dilution of the bipolar concept is extreme.)
