> JimmyHigh dose Effexor is your best bet for TRD. The only things more effective than high dose Effexor are MAOIs and ECT. You can go higher than the legal PDR max of 375 mg, its done all the time. Providing your blood pressure can tolerate that high a dose.
If you get to 375 mg Effexor and still arent out of depression, add Remeron to it. Thats a potent AD cocktail for TRD known as "California Rocket Fuel." And you could even add Lamictal to it for even higher efficacy.
Forget the PDR (unless you have liver, blood pressure or seizure problems) and just push the dose as high as you can take it.
I wish the drug companies would create liquid injectable forms of these antidepressants, like theyve done with anti-psychotics. This way people with severe depression could get antidepressants injected rather than taken orally, bypassing the gut and liver and all that absorbtion crap. Getting into your system directly...probably would be more effective.
