"...For paroxetine {withdrawal}, all but two patients tapered by 10 mg/day each week, one patient by 15 mg/day each week and the other stopped at 20 mg/day abruptly. The onset of symptoms occurred no later than 5 days and usually 2 to 3 days after the last dose of medication, but on occasions, it occurred during taper, from paroxetine at 20 mg/day to 10 mg/day in two cases and from sertraline at 50 mg/day to 25 mg/day in one case. In patients who were restarted on their SSRI while still symptomatic, the symptoms resolved within 24 hours. Nightmares and insomnia after stopping clomipramine were also relieved on the second night after commencing venlafaxine in one patient. However, two patients who tried to control their symptoms with additional oxazepam, 15 to 30 mg/day, or diazepam, 20 mg/day, had no relief, and two patients who commenced moclobemide, 300 mg/day, 7 days after discontinuing paroxetine had symptoms persisting for at least another week..."
