My insurance co. that I got reinstated with via Cobra, uses MerckMedco for their prescription service. I received a partial list of formulary drugs, and thought it might be interesting to share. (P.S. I'm just the messenger here!)
Symbols: $ lowest relative cost, !!!!! highest relative cost, ++ Brand preference exists, use designated brand.
Where a generic is listed alone, it generally means the Brand Name version is non-formulary so generic must be prescribed.
