ryan_s - For pure bipolar I, lithium is still the most effective treatment. Sometimes augmentation with an atypical antipsychotic (eg Risperdal™ or Zyprexa™ or Seroquel™) or with one of the antiepileptic mood stabilizers (eg Lamictal™ or Neurontin™ or Topamax™ or Epival™/Depakote or Tegretal™) is necessary. The choice of augmenting medication really depends upon one's response to lithium (ie non-responder or partial responder) and upon the presenting symptoms.
Zyprexa or Risperdal are being used more during acute manic episodes, for quicker control and they may be adjusted or discontinued once the lithium kicks in.
