Scott,
I am greatly relived to hear that you are in the land of the living, but sorry to hear that you are feeling so lousy. I don't post either, but there are lots of reasons for that.
It's been two weeks since my last suicide mission, so perhaps (tentatively) there is hope.
Have you considered oxcarbazepine (Trileptal)? It is a keto-analogue of carbamazapine (Tegretol). The modification to the molecular structure prevents it from being metabolized by oxidation to CBZ-10,11-epoxide, which is the metabolite that is thought to be responsible for much of the toxicity of CBZ. OCB is metabolized by conjugation rather than by oxidation, and largely excreted by the kidney. See e.g. Lloyd et al.
We are adding it on very slowly to a cocktail of Lamictal, selegiline, Aricept, Klonopin, T3 and T4. My docs are adamant that adding ADs or stimulants will only make the depression worse. (The dose of Selegiline is low). I didn't tolerate Tegretol at all because of side effects. I do find this a bit sedating, but we are taking it very slowly.
Just a thought.

Hugs and prayers,
We've missed you.

Chris A.
