Others know more about this than I do but I thought that if a person had a good mood improvement with an SSRI but the side effects are intolerable, then the person should be either switched to another SSRI (or other serotonergic med) that wouldn't have those side effects or have that original SSRI (Paxil in your case) augmented with another med that would ameliorate the side effects. For example, Wellbutrin is often added to an SSRI to lessen fatigue and sexual side effects. Reboxetine combines well with SSRIS. Another option is some of the newer meds that combine serotonin and NE action. See D. Bob's Tips section under the search terms 'SSRI' and 'side effects' for more ideas.
