Judith C. gave you some good advice. Calm down, take a deep breath and simply pick up your pen and fill out the forms. Trust me, once you get started, it won't be as difficult as you had imagined. My oldest son, who is physically handicapped, hadn't been reviewed for *years* and all of sudden they sent a packet of forms and wanted information, etc. Good gravy. They were wanting bank statements and things that we'd pitched a long time ago. I searched and searched until I found everything he/we had and I simply wrote and told them that for being a pack-rat I hadn't done very well at keeping his "papers." Bottom line was that they accepted what we provided and simply made an adjustment in the amount he receives due to his income. No big deal. I realize that mental/emotional disorders are the hidden ones and can be difficult to prove, but considering you've already been approved once should help pave the way for the continuation of your benefits. In other words, "Just do it!" Good luck - I'm sure things will turn out just fine.
