Vesper, you need not do either. You must speak your pain, frustration and hopelessness to the ones who are there to help you. Let them know in no uncertain words that you need a hand to pull you from the deep pit. Tell them that you are in trouble. Wait in the ER for three hours knowing that you will be attended to, you will not be sitting there forever, you will not be left out in the cold, alone and helpless.
I always feel guilty when I get attention only because I said I was on the edge of doing something extreme. Almost like the little boy who cried wolf one too many times. But, I don't want you to feel that way, it is okay to cry wolf, we can see the beast outside our own doors and can surely see it trying to sneak through yours.
Please don't look at it as an all or nothing scenerio. There are options that are hiding in your blind-spot. Take door number three instead, get yourself back to that ER and sit on your hands and wait. It may be painful, it may be incredibly slow, and you may have to repeat your excruciating journey; but you will get help. We don't want to see you fall into the darkness again, we care about you.
You will be in much better shape to gain real effectual help if you can hold on a little longer. The self-inflicted damage is still damage, and still creates wounds, not just on the flesh but on the heart and spirit. You will be in my prayers and thoughts Vesper!
