Amanda - I have seen some good things with Topamax, but it takes some time to get up to a therapeutic dose. You have to start low and raise the dose slowly to avoid confusion and other cognitive problems (eg difficulty with concentration, such as simple math problems). Dizziness can be a problem, especially at the start of therapy. Also drink plenty of fluids throughout the day (water or juice) to avoid the production of kidney stones (minor side effect - 1.5% risk). The most notorious side effect is weight loss in 10-20% of people who take it. People who are overweight tend to lose more weight than those who are underweight.
Warning - Topamax should not be used to reduce weight only. It is a potent drug and has effects on mood, as well as many side effects. Most weight loss is only in the first 3 months and only 10-20% of those who take it lose weight. To lose weight eat a healthy diet AND exercise.
Topamax seems to work well for some manias, bipolar depression and ultrarapid cycling. It is usually used in combination with another mood stabilizer.
