Does the shaking happen only when you sign your name? Or does it happen whenever you write longhand?
If it's whenever you write longhand, I can certainly sympathize -- I have a hard time writing by hand at all, due to a combination of CTS and Wellbutrin, and it's damned embarrassing for me.
If, on the other hand, it happens only when you sign your name, it may be a weird thing that happens in your brain -- I'm looking for it in another window just now -- that Scott Adams just wrote about in his newsletter. He's got it, and it keeps him from drawing by hand. focal dystonia -- that's what it's called. I have a headache, though, so I'll leave you to google it. Sorry.
While I hate to suggest that you might be ready for another dx, at least this one is an interesting one, eh?
with much affection to you.
