It talks about the legal liabilities doctors face in terminating that relationship, and it might give you some insight into what you're going through right now.
It's not right that doctors have to consider these aspects in treating patients, but it is a reality for them. I'm betting that if you talk to him about it, he'll be open to exploring your options. (And pay special attention to the little instructions at the end: your doctor really should help you find appropriate care. It really is part of his job.)
