Very many people experience the same problem with the SSRI medications. You will often find it refered to as poop-out. I have yet to see anything that points to a firm understanding as to why it happens. For me, augmentation of the SSRI with Naltrexone has been miraculous… like a rebirth after 35 years of depression. It is not a well known augmentation and your friend’s Dr may need some background to be willing to try it. Here are some links to more info.
