From what you say of your symptoms and the medications you have tried, it would seem to me that the best thing to try might be one of the well-known serotonin reuptake inhibitors: fluoxetine (Prozac), fluvoxamine (Luvox), sertraline (Zoloft), or paroxetine (Paxil).
These have a very good track record in obsessive/depressive states, whereas the various"drug cocktails" you have tried so far do not seem to have been very serotonin-oriented. If your anxiety is severe, and especially if you have insomnia, I would perhaps suggest trying paroxetine first since it's the most calming, but the others are probably just as effective (or even more effective) in the long run.
