Would you even add a sixth psychmed here?

Posted by Lamdage22 on November 18, 2020, at 5:51:47

Does this even make sense? I have a few candidates, however no one to prescribe them. You can see my cocktail below.

Phenytoin. Memantine. Stuff like that! Is this a good or a bad idea?

id really 1st see about a slow and steady reduction in neuroleptic load, if possible and OK with a good doctor. to that end...

if its OK, then maybe an as needed sedative of whatever sort, during the dosage reduction and for a time after wards, while your brain readjusts?

that's just me. not to nitpick your treatment providers or whatever, but you seem to be on a relatively high doses of 2 different newer neuroleptics. every day.

i read a little tidbit somewhere that animal studies had suggested that using lamictal with neuroleptics reduces D2 upregulation, which I would think might make it easier to stay on a steady no (no escalation) or even reduce a bit (maybe?), but that's just from memory and it was just animal data, not anything from people.

other thought...random reading, here and there, on 'tardive psychosis' (not trying to scare you...applies to me, too, plus however many people are on the neuroleptics for...whatever indication...) has me tempted to ask for some kind of anticonvulsant, but...

ugh. even lamictal, trileptal...not without adverse effects, risks, drug-drug interactions. phenytoin? why?

Memantine can be great for depression, if you take enough (otherwise it is pretty much without effect). In fact, it proved *too* great for me. I got manic and ended up in the hospital.

Well if I took less Neuroleptics, I'd be unstable. I am reducing it every couple of months. I plan to ditch daytime Seroquel because it doesn't at all help cognitively. Zyprexa is much better in this regard.

I seriously doubt that I could become hypomanic on this tranquilizer dose. Low dose Phenytoin may help social anxiety.

Hmm. Well, the "I don't need my meds anymore" hypomania should be avoided. That is a recipe for disaster. I did that once and hence I am on this hefty tranquilizer dose.

Well, depends on how much you need your meds.

It is more of a "gonna use memantine again" type of hypomania. And you become very playful, impulsive, easily amused, enthusiastic, wakeful, credulous, and more. I was laughing all the time just from YouTube :D

It sounds cool. There is much more evidence for Memantine than Phenytoin. But my Psychiatrist won't be thrilled I imagine. At some point you don't know anymore which med is doing what.

What are the usual side effects?

At first, you feel weird, until you get used to it.

Then, there is the (hypo)mania with its regular consequences. With the playful, impulsive and credulous features, you easily develop funny ideas that may border on delusions. I imagine that's not a good thing if you are already prone to psychosis.

Probably not. Unless you can steer it with dosage so you are euthymic and not hypomanic.

That kind of dosage adjustment seems tricky in my experience. I don't recall experiencing euthymia in the absence of hypomania. It was either "too little" or "too much".

Mania can be very destructive. Even Hypomania

Having quitted the meds during hypomania, I had to quadruple my Neuroleptic dosage! So I got the exact opposite of what I wanted! So please no hypomania!
