Hi Ed,

You responded to my post above, but I thought I'd start a new thread.

My rash is not doing much better. I saw an online doctor (major backlogs because of COVID) and this was pretty much useless. They prescribed Loprox (ciclopirox) which has done nothing. The doctor could only look at pictures and I really have no firm diagnosis.

I am pretty sure it is tinea corpus, but it hasn't responded to any OTC antifungals (clotrimazole, miconazole, tolnaftate) and now ciclopirox.

The problem is that the rash creates a yellow sticky goopy coating which is virtually impossible to remove. I literally scrape away at it with apple cider vinegar and a green scratchy pad (or fine sandpaper) and then the goopy coating is back the next day. Anything I put on it topically is just not getting to it. I likely need oral antifungals (if it is tinea).

If you have any insight shoot. I'm seeing a dermatologist next week, but again its 'online' (useless) and they likely won't prescribe any oral antifungals (without lab work?)

Link I'm not Ed. but are those holes in your shins? Almost looks like a farmers sunburn? Will google skin conditions you mention here and see if under images if I see anything like what your pictures show.

Hi Philippa,

I've had it for over 2 years (has been growing and expanding).

Those holes are the places where it originally started. It eats down into the skin and then pours out a bunch of goopy yellow crap which (I assume) contains more spores and allows it to spread.

I really have no idea. It's nasty and nothing seems to work.

Yeah. It sort of resembles that. It did start in a ringed pattern.

Possibly, but I don't seem to have the risk factors. I'm 37 and have no heart or vein issues. I don't think I have any clots (I eat very well) - no cholesterol or blood sugar issues.

Link that is something that definitely needs to be cultured as sounds more like bacteria. Can you go to an ER in a hospital?

If you can't get a prescription for oral antifungals, do you think you might be able to acquire them without one?

I asked Ed in your prior postings if biologics would help. It seems that a systemic treatment might be more effective than topical. Anyway, I would like to hear Ed's opinion on this.

The difficulty is in seeing a doctor face to face. If I had benefits, then a trial and error process of $thousands in various creams and powders would be fine. Because I'm paying out of pocket, I really want to get the diagnosis right.

At this stage, I am fine with taking oral antifungals, but I don't think that an online doctor will prescribe them (the last doctor said that bloodwork needed to be done first, and they might interact with other meds).

To be honest, I'd stop all other meds, if it was necessary to get rid of this.

Yeah, the first doctor seemed to think that a fungal infection so widespread wasn't likely. He said it only usually happens in immunocompromised individuals.

I had a really stressful moment about 2 years back, and employment related stress since then (prospect of job loss) that psychologically I haven't been able to surmount. I've really been feeling worthless for 2 years.

I wouldn't hesitate to think that my moods and my immune system are linked. When I have something growing on my body that I can't get rid of, it makes me fear that things are growing inside my body that I can't get rid of.

Fortunately, the holes have healed to some degree, but the rest is still there. The pictures are about 3 weeks old. The rash itself is more itchy than painful. It has kept me up at night.

I wish there were something I could say that would make you feel less worthless - and I can. But I don't think you would be easily convinced. You have a warm heart, a sincere voice, and a powerful mind. You are usually right about everything.

Many people fall into the trap of defining themselves by the job they have and the size of their bank account - even the car that they drive. It's hard not to. At some point, I had to make the decision not to do that anymore. I felt my calling was to be a MD or PhD in neuroscience. I lost my ability to read, learn, and remember when I was 20, so I sold cars instead. I don't know why the Universe chose me to experience life in an altered state of consciousness that was both painful and numbing. It was not something I could control, and it wasn't my fault. Being ill wasn't a choice, but how I approached life was. I realized that the most I could possibly expect of myself at any given moment in time was to use all of what little God gave to me to work with.

Whatever works for you. One thing for sure - self-loathing won't.

Be merciful on yourself.

Cognitively I know what you are saying. Actually while under the influence of medical marijuanna (about the only time in my life) I can more easily accept my worth independent of traditional indicators.

That being said, I suppose I've been a workaholic as a way to avoid dealing with internal issues. This perhaps has made me addicted to only viewing myself in terms of my job.

Hi Link,

I feel for you. The skin condition looks terribly uncomfortable.

It certainly could be a form of tinea corporis, but it's not showing the classic patterns which can be used to make a 'visual' diagnosis. I can see widespread erythema and lichenification due to scratching. I think you probably do need to be a lot gentler with it and avoid abrasive rubbing.

The small abscess-like areas containing pus could have been a kerion. This may be what left the 'pits' after they burst.

It's obviously reached the point where you need to see someone about this urgently... Get some skin scrapings done to send to the lab, observe the skin under a Wood's lamp (if available), and potentially do a diagnostic skin biopsy. Some routine blood tests would also be advisable.

Any fungal infection that widespread would be treated with oral medication, usually terbinafine, less often itraconazole.

If it's not fungal, you urgently need to know what it is so you can receive appropriate treatment. Virtual online appointments just aren't appropriate. I'd literally be turning up at the ER if I really couldn't see a doctor after such as long time.

How are you feeling? Any progress with seeing a doc?
