I need antifungals - may f-up my meds?

Posted by linkadge on May 9, 2020, at 16:03:46

I've been battling a ringworm (tinea) infection. It has spread (over 2 years) over my entire legs and on my arms. I had been trying to self-treat it with various herbs and OTC antifungal creams with are doing sh*t (worse than nothing). I was afraid to go to the doctor for oral antifungals because I understand they can f*ck up med levels. Sorry for the swearing, but this rash has made me very pissy (how insidious it is).

Has anybody here successfully treated a fungal infection while still taking psych meds? How did you do it?

Ketoconazole notoriously interacts with pretty much everything, so better be careful with that.

I took terbinafine (Lamisil) while on at least selegiline.

https://www.ncbi.nlm.nih.gov/pubmed/10460803
"The potential for terbinafine interaction with other drugs is predicted to be insignificant with the exception that it may inhibit the metabolism of CYP2D6 substrates."

Hmm.

Were you taking oral terbinafine?

Did it work?

I think there is a connection between my mood at this rash I have been developing (over 2 years). I'm wondering if there is an immune link. My father (no depression) got the exact same infection. It is much less widespread and is seemingly gone now.

I stopped weed (reluctantly) a few days ago to see if (perhaps) it was supressing my immune system's reaction to this. We'll see.

Yes, but I've also used it topically.

Better than ketoconazole at least.

My problem seemed to be some kind of combination of seborrheic and atopic dermatitises (including dandruff). The resolution of this syndrome coincided with my trying a combination of oral salbutamol (Ventoline) and cetirizine (or other non-sedating anti-histamine).

Seborrheic dermatitis is more common in Parkinson's disease, and some doctors reported that it occurred in patients treated with chlorpromazine.

Yeah, this started about 2 years ago. I had started using pot about 3 months earlier (which may have increased my susceptibility - not sure). I'm pretty sure it's ringworm, but it has been resistant to clotrimazole, miconazole, tolnaftate, apple cider vinegar, tea treat oil etc. It's spread across my entire leg, and not on my shoulder etc. Lithium may be drying out my skin too (making it more susceptible). Not sure, but it's brutal. I'm seeing a doctor via computer today. Who knows.

I always thought that people with 'fungus' were unhygienic. I've been washing and disinfecting like I have OCD, and it makes no difference. I am very sympathetic now to people with skin conditions.

Saw a doctor today. They prescribed Ciclopirox cream. The problem is that it costs $60-80 a tube. This would last me about a week. Not happening. Not with no income.

Back to suffering.

The world is a wonderful place if you have benefits.

Pity. Have you tried piroctone olamine (aka. octopirox)? It helps me keep the dandruff away, but I don't know about ringworm.

Yes. It is hard to appreciate when you're depressed. There are many things to be grateful for, such as eyesight for example, but I always forget.

I can barely see because my glasses are so scratched and the prescription is over 10 years old. I can't afford a new one because, again, I have no coverage. I have repaired them several times with glue, tape, and heat shrink tubing.

As far as octopirox, the rash is so widespread that I go through a 30g tube of OTC antifungals in under a week. It has completely covered my legs and now my arms and shoulders. I can't afford another $50-100 a week with zero income. This is especially true for something that may not even work or may take weeks or months to show any improvement.

The rash is so widespread and deep I NEED an oral antifungal (cheaper and likely more effective). Even if I go to the ER, they're likely going to send me home with useless steroids.

Canada's healthcare system is a broken. It works for people with benefits. It doesn't for people who don't.

I didn't have anything so serious as that. But for decades I suffered repeated cold sores and chronic nail fungal infections.

These went away forever when I went on Rei Shi. It's not magic in that it didn't prevent my skin cancer as well, but I remain impressed.

What about Lamisil cream?

That's what I was hoping for. Its not OTC in Canada so I would need a prescription. The problem is the coverage of this rash is so extensive that I'd go through a tube in a few days.

I've never had skin problems in my life. I'm not sure how I managed to get such an extensive infection. Honestly I see a connection with my mood. I thing my depression has compromised my immune system.

My son took oral terbinafine (Lamisil) for toenail fungus in March and it opened up a whole can of worms. On third day of therapy he developed chest pain, shortness of breath, insomnia, loss of appetite (but not loss of taste), fatigue, and anxiety. And over two momths later he is still has loss of appetite and occasional chest pain. Covid was ruled out by PCR and antibody testing and chest xray and labs (liver function, hematological, etc) look great. So by exclusion, terbinafine might have triggered anxiety and panic attacks. Lamisil cleared up my husbands toenail fungus fine with no side effects, but when you get side effects, watch out. Panic disorder runs in the family, but my son never had problems before. Terbinafine may have unmasked it. Good luck!

How's the rash doing?

In the UK, small tubes of Lamisil cream is OTC... But widespread infections are typically treated with larger pack sizes on prescription, as generic terbinafine. You would probably receive a few 30g tubes if the infection is very widespread.

Typically, topical terbinafine is very effective for tinea on most of the body. Infections of the scalp and nails require oral treatment, however.

Shame you aren't here in this instance.... because this would be sorted in no time.

Which creams have you tried so far? I assume at least clotrimazole or miconazole?

I started a new thread at the bottom (called to Ed_Uk). This rash is brutal. I don't know if I should walk in the ER. I'm not getting any help or relief.

Ed - are there biologics that would help?
