How is everyone?

Posted by PeterMartin on March 28, 2020, at 0:47:03

On a scale of 1-10 (10 being best), how is everyone doing?

I had a really bad few days that have to be medication w/d related. I stopped Desipramine and then Nuvigil within the past two weeks.

Today for me was one of the worst days in a very long time.

I was a 1 earlier. Probably a 3 now. Hoping I wake up back to my old self :-/

Marplan 40
Lamictal 200 (took 250 today and may stay)
Metformin 1500
Seroquel 25 sleep

I'd say about 7 for me. In terms of just mood, maybe 8 (10 would be at least a bit of euphoria). In terms of apathy, maybe 6. In terms of cognitive impairment maybe 6. In terms of anxiety 10! In terms of psychosis 10.

That sounds pretty solid. I am fluctuating depending on what news I hear!

I'm still fighting a very deep hole of depression (very likely due to ceasing Nuvigil 4 days ago, and Desipramine a week or two ago).

Took 300mg of Lithium today just to try and take the edge off. It worked a little.

I need to get over this.

Thanks everyone else for posting your current situation. Knowing others are out there helps during times like these.

Hi.

I have had an awful two years. It is more than I can put into words. I spent much of that time at about a 2. Right now, I am about a 3, but I was worse earlier in the day. I experience one or more mood shifts a day. Bad to worse to bad again...

I have been at 60 mg/day of Marplan for about a week. I don't know what to expect. How long can it take for Marplan to kick-in? Why are you not taking 60 mg/day?

I hope you can hold out until your condition improves.

Good luck.

It's good to see a post from you Scott.

I bumped up to 50 today. In the past 30 to 40 of Marplan was enough and since I'm paying out of pocket I've tried to stay there.

I started doing really well the day after I added Desipramine but I almost think that was a timing fluke. I probably should have held off. I'm hoping once this w/d from Nuvigil (and perhaps even Desipramine or Ritalin a few weeks before that) will subside.

Mostly anxiety tonight after an ok afternoon.

I also kicked my Lamictal up to 300mg at least for today. I need something. The world is so depressing as it is I don't need the feeling in my legs.

Hoping after a good nights sleep I'll wake up much better. Today was my 4th day w/o Nuvgil.

Marplan usually takes me 14 to 21 days to get a response. Do you have a bp cuff? A
drop in BP is usually a good indication (for me) that it's starting to work. Mine now is 110/75ish somethimes lower - without Marplan (or another MAOi) it'd be 125/85.

Maybe a 5. Just feel paralyzed with whats going on. My parents are elderly and Im very worried about them.

My withdrawal from Nardil is not going as well as the last time I discontinued it. Taking 3 1/2 tabs per day and having serious nightmares, and waking up seeing things. I actually screamed in the middle of the night last night at waking up to strange people hovering over me.

Should I up my Olanzapine? At 5mg along with 5mg melatonin at night. Not sleeping again. What seems strange to me is that I actually sleep a lot better at 60mg of Nardil.

Hope you are feeling better soon, I know Id like to see you get a good result from Marplan. I have a bottle on my shelf.

Thanks Jade! Wish I had some advise for Zyprexa dosing but I haven't taken it in many years.

Thank God I finally found myself again today and the anxiety lifted. It must have been withdrawal from the Nuvigil. I found a few posts last night on Reddit and drugs.com from people who said w/d from Modafinil/armodafinil is absolutely possible and it one guy specifically said it took him a week to -start- feeling normal again. I took my last Nuvigil 250 exactly a week ago tomorrow (Tues).

W.o. the anxiety this afternoon and evening I was doing projects I like to do again. I definitely do believe Marplan is helping me a great deal (at 40) and there's still room to go up. I just need to go a month or two w.o. adding anything. I think I shot myself on the foot adding Desipramine right around the 5wk mark Marplan. I had noted "10"s for mood the day after starting Desipramine (only 25). So likely Marplan was just kicking in and I clouded the response.

I had to stop Desipramine due to incredible sweating even at 73 degree room temp. I'm super happy to be over the anxiety/cold depression I'd had the pass week or so since stopping Nuvigil.

I'll try to update sometime soon.

Good luck to you and your family w Covid. Scary times all around.

All good now. Bumped up my Marplan to 50 and my Lamictal (Unichem) to 300. The w/d from Nuvigil also finally dissipated.

I hope everyone is managing these strange days ok.

Hey, glad you are feeling better! My brain has shut down on me, not sure if its due to watching too much news, or med issues. Anyway, keep us posted as to how you are doing on Marplan, at some point Ill be joining you :)

Stay well,

right now, about a 4, things are moving slow, i've been trying to start projects and then just totally lose the energy. Have a mood swing, or just get in a bad mood while im doing an activty. Most people don't get that way, like when your doing work, you keep doing it. Reading about new age stuff, ascention, its not true, there is no asention, you simple are devided where you going to go. not suppost to dabble babble in that. Learn to have a bullcrap detector when im reading some of these new age christanity teachings, and im serious htey are popular and are deceiving many people.

Anyways, i'm.....blank, and feel drift. Prolblems and solutions, and i don't feel i have a machine or paddle to get me going in the right direction...

thats all

Marplan is very expensive, probably because the volume of sales is lower than Parnate or Nardil.

> I started doing really well the day after I added Desipramine but I almost think that was a timing fluke. I probably should have held off. I'm hoping once this w/d from Nuvigil (and perhaps even Desipramine or Ritalin a few weeks before that) will subside.

I'm confused. Are you still taking desipramine?

> I also kicked my Lamictal up to 300mg at least for today.

I am much better on 300 mg/day of Lamictal than on 200 mg/day.

> I need something. The world is so depressing as it is I don't need the feeling in my legs.

Is the legs thing a side-effect? Things are very different in the world today. It's eerie.

> Marplan usually takes me 14 to 21 days to get a response.

I am not doing well. I have been on 60 mg/day for about 2 weeks, and I feel worse in some ways. I feel pretty weird and have a tendency towards suicidal thinking. I haven't posted much over the last 2 years because of how poorly I am doing. My only choices right now are to go back to either Parnate or Nardil. I have more mental energy and function better on Parnate, but there is little to no improvement in anhedonia and motivation. Nardil is much better in that regard, but it doesn't seem to last. I decided to come off of Parnate 2 years ago because I didn't want to live the rest of my life that way. Unfortunately, 2 trials of Trintellix caused me great harm, and it took a long time for me to recover from them.

> Do you have a bp cuff? A drop in BP is usually a good indication (for me) that it's starting to work.

Yup. In fact, one of my doctors in the 1980s measured the difference between my blood pressure sitting and immediately upon standing. A 10% drop (orthostatic hypotension) was an indication to him that the dosage was high enough.

I definitely felt better early on with Marplan, but it was episodic. Right now, it is doing more harm than good, and I am reducing the dosage. I will probably discontinue it.

How would you go about switching from Marplan to another MAOI?

Thanks.

That's very familiar to me. Do you find yourself becoming motionless and staring into space after awhile once you start an activity? I've been dealing with that my whole adult life.

With me, there is no such thing as having projects. I don't have the motivation or the energy. When I do respond briefly to treatment, my mind becomes more active and I begin to do things. Otherwise, I spend the entire day lying on the couch and watching TV. Mornings are better for me, and I spend some time on Facebook to distract myself.

I would go straight from one to another. Less adventurous people would cross-titrate.

Hi, UD.

> I would go straight from one to another. Less adventurous people would cross-titrate.

Thanks. I appreciate your input.

I decided to take only 10 mg of Marplan yesterday. I took none today. I actually feel better. It seems that Marplan was increasing suicidality rather than reducing it. Also, I have been experiencing great fatigue in all of my muscles. Not so much today. I hope it was a side effect.

> What about clorgyline? Years ago, you said it was good. Why did you stop it?

Clorgyline is easily the most powerful MAOI, and was considered to be the most powerful antidepressant in the world when I was being treated with it by the NIH in the early 1990s. However, the NIH stopped giving it to their patients because there were some fatalities involving cardiovascular function. I stopped taking clorgyline before that, though. My response was incomplete, and they wouldn't add a TCA to it.

Per unit of weight, or what do you mean?

I think you can get near 100% MAO inhibition with the other potent MAOIs too. Correct me if I'm wrong.

Wow, I'm so sorry to hear Marplan didn't work out for you. I've actually gotten much much better in the past two weeks. Like best in a couple years since I stopped Marplan the last time. I'm sure bumping up Lamictal helped a bit also, but Marplan definitely works Wells for me. I have how we all can be so different w these medicines. I'm sorry if my success had anything to do w you retrying Maplan again.

If you are planning on switching back to Nardil or Parnate I personally wouldn't wait all that long. Particularly if you go back to Nardil (hydrazine definitive also). I've gone Marplan to Nardil to maplan to Nardil w.o. any days off.

My current meds which have me optimistic and productive (computer work I do):

Marplan 60
Lamictal 300 (Unichem verified by Valisure)
Metformin 2000 (Mylan 500x4 IR verified by Valisure)
Seroquel 25 Lupin for sleep

I'm sorry to hear that Scott. Chronic depression is a truly distressing, disturbing, crushing illness. Looking at my mood spreadsheet, my average mood is about 4/10 over recent years which translates on my scale to "strongly want to be dead most of the day". The last 2 weeks have been 2/10, which I'll leave to your imagination how bad is.

Fortunately since returning to 30mg Lexapro, I'm more back to 4 or 5. Yay. FTR my stack is :
35 Percocet for pain, 2 Lorazepam for OCD ad hoc, 200 Provigil, 30 Lexapro, 2.5 Abilify, 30 Ritalin for depression, 60 Adderall for depression, 40 Fetzima, 45 Buspar for OCD, 300 Lyrica for pain, 225 Trazodone for sleep
