PD NOS

Posted by capricorn on February 27, 2007, at 13:34:01

That's my latest dx. Past dxes-schizophrenia, schizoaffective/bipolar, dependent PD.

Can anyone enlighten me on PD NOS? Is it one of those
'waste paper basket' dx?
A way of saying we are not sure you are really ill but just in case?

'They think you're a fake' keeps going though my head :(

I like the NOS category.

I like to think I'm too complicated to fit neatly in any simple DSM category.

NOS makes me feel fully wonderfully human somehow.

PD NOS is given when you overlap one or more PD's but don't technically fit the complete criteria for a specific PD - it doesn't mean you aren't "ill" or they think you're faking - a better term would be complex PD - sometimes a pdoc will defer a definable PD until the axis I is under control and give a more general dx of PD NOS - it all depends on your particular situation.

I do not believe it is a *trash* dx as Phillippa said.

I'd ask your pdoc why he/she gave you that specific dx.

He's seen me three times now for a total of about 40-50 mins.
First time put me back on zyprexa at lower dose but wouldn't give me AD he said because of possible adverse effect on mood.

This time when i pressed him re the PD NOS. He somewhat reluctantly offers up 'obsessive neurosis' to flesh out the explanation.
Tells me quickly 'but it's not OCD'.
Funny how a google search turns entries showing it's an archaic name for OCD.
It's way off the mark. Almost as off the mark as when several years ago i was told there was only Dependent PD on my notes(different pysch). At that time i was on lithium(had been on it 20 years)and blood test forms said bipolar or schizoaffective.
Next time i had an appt which was 3 weeks later(it was a locum consultant)and questioned her i got told 'All you need to know is you have an illness with a psychotic component'.
I thought to myself 'what the expletive deleted'.

I think it's true what they say about pdocs. Take 6 of them give them each the same dummy patient complete with notes and there's a good chance they'll assign 4-6 different dx.

Then you're supposed to trust them.Who would trust
half a dozen oncologists who told you that you were respectively suffering from lung, brain, stomach, and liver cancer?!

NOS (not otherwise specified) tends to give more leeway with HMO's as to the treatment options they will allow or not cause a big fuss over.

I wouldn't waste time thinking about diagnoses.
Not from western medicine.

These things were better understood a thousand years ago.

A spot of involutional melancholia here.

Nothing of the sort. It proves your doctor is doing his best to not label you.

One of the criteria upon which a personality disorder rests is that it is a stable group of behaviours/thoughts over time. I know that PD diagnoses get tossed around with some abandon, but if a doctor follows the diagnostic guidelines, a PD should not be diagnosed until after 8 weeks observation (or is it 10 weeks?). The idea is to exclude state (temporary emotional environment), and observe trait (stable) characteristics.

Your new doctor is planning to make his mind up for himself, rather than relying on the opinions that might already appear in your chart. Moreovoer, he's biding his time, to allow you to reveal yourself to him.

As another poster already suggested, the diagnosis is a billing code for health care management people. It makes sure he gets paid, and you get cared for.

Best,
Lar

I failed to consider that your doctor might have been seeing you for a while. If that's the case, it's because you're unique and special. People don't always, or even often, fit nicely into the little diagnostic cubbyholes of the DSM. It's easy to imagine straddling lines, especially between similar diagnoses.

I long ago decided that I'm not strange, I'm eclectic. A tad eccentric, perhaps. Ya know?

Lar please don't use the word eclectic as Danielle Steel wrote a whole book and the whole book had nothing but that word it it. I could scream just hearing it again. How bout unique?????Love Phillipa
