Some begin to belief that they have something or they may have symptoms but in fact they have nothing but accumulated stress that lowers the threshold such as family problems, underlying physical diseases or setbacks. Or some have curiosity and hypochondriac tendencies.

If accumulated stress (rather than a genetic predisposition) gives you a heart attack, is it any less treatment emergent?

Not sure what you're getting at?
I.e. people who seek treatment often have real symptoms that significantly impair their ability to function. If it gets to this stage, then who cares if it is "accumulated stress" or "hypochondriac tendencies".

stress and psychology are often real problems. Hypochondria is classified as an anxiety disease and i think the term used now is illness anxiety disorder.
psychopaths maybe won't get symptoms.

We're not really here to diagnose people.
Are there people taking medications, who would benefit more from other interventions? Yes.
Are there people not taking medication, who would beneift from medicatons? Yes.
