Counteract agitation and anxiety from Parnate

I'm really on edge from this 60mgs of Parnate but can't go off it because I'm out of options for my hard to treat depression. What can I take to counteract the agitation without impairing my cognition and turning me into a zombie? I increased my Lamictal from 100 to 150 but it's impairing my cognition and slowing me down and I don't want to go up any higher on the Zyprexa 5mg because that REALLY slows me down . I'm already drinking ten cups of coffee a day . Ugh.

I used to take Parnate and I'm pretty sure the agitation is from a reduced ability to handle stress from the sleep deprivation that Parnate causes. If you can find something to help you sleep, you may be able to handle stress better and will have less agitation.

Can't handle the agitation though. Is it a manic dysphoric mixed state or the fact that I don't have racing thoughts or trouble sleeping make that not a possibility ?

I don't know what I have!

You only need a certain number of manic symptoms to qualify for a manic episode so you don't necessarily need trouble sleeping or racing thoughts, when a medication is involved it is hard to say what is a manic/ mixed state and what is medication-induced or side effects. I wouldn't worry too much about the etiology though.

what about a different antipsychotic that isn't as sedating as the Zyprexa? like Latuda or Abilify? I believe in Italy Parnate only comes in a combination that includes trifluoperazine probably for the issues you speak of.

or a benzo?

I think you should reconsider the parnate and perhaps go with a different MAoi...

It's been so helpful in other respects . I hadn't been able to get out of bed for months on end before I took it. Rather try to counteract the side effects then risk going of it.

I think that's a smart plan of action -- I myself was wanting a switch from Abilify but my doctor is advocating that I stay on it for now because my bipolar -- at least the mania/ mixed side-- is finally under control. So I totally understand not wanting to switch if you've found something that's actually doing it for you.

It's always good to have back-up plans too, or at least to know there are other options. Have you taken Marplan or Nardil?

No the only other maoi I've taken is Emsam which pooped out pretty quickly and just not that strong ."MAOI light" I had one doctor call it.

Oh okay, you have some options then. I have taken all 4 and take Marplan currently. I also experienced some agitation on Parnate so I know what you're talking about. Nardil and Marplan, in my opinion, are less likely to cause this. But I would put those 2 on the back burner if you're doing well on the Parnate as you have options to manage it. If you choose to try them, Abilify or Latuda may augment your antidepressant response as well. I'd say that risperidone/ paliperidone (less sedating that risperidone) may help with the agitation too.
