I have had some bad insomnia over the years. Most of the time, it responds to a single agent. However, I have found the combination below to have never failed yet in really bad situations.
1) melatonin time release (2.5mg ish)
2) remeron 2mg
3) lorazepam 0.25mgThe doses of each are very small, and there is usually very little, next day impairment.
I take the two about 15-30 min before I want to be out.

I have always found extremely low doses of remeron...anything from 1mg to 7.5mg...are more effective than higher doses when treating insomnia. I only used loraz a couple times but I remember it was a smooth soothed feeling perfect for sleep.
I like the way the synergy of the three ingredients has allowed it all to work with very low doses of each.

I think a crumb of remeron is the best med for sleep ever, and it worked even better with the benzo I was on.
