And now its Nardil

My anxiety hadnt gotten any better on 45mg of Mirtazapine for a month, so last night me and my doctor decided to try Nardil

I'm cutting down the mirtazapine over 6 days, will have three days washout, then start Nardil

My regimen will then be

Nardil 30mg twice a day

Nortriptyline 75mg at night

Risperidone 1mg at night

Xanax 1mg three times a day

Nardil is about the last resort for me, so I'm putting a lot of hope into it!

Next step will be to change from Xanax to a longer acting benzo like Valiumif things go well

It is a logical thing to do.

If things don't work out with this new regime, I doubt you have reached the end of the line. I like your chances with the Nardil, though. Since you will have nortriptyline on board, be prepared for the possibility of increases in the magnitude of hypotension and urinary retention. Keep the Florinef (fludrocortisone) and Urecholine (bethanechol) handy. I found that these side effects mitigated somewhat with time, but did not disappear completely.

I had some urinary hesitancy in the begining with renoxetine, but not with nortriptyline - it did ease off after a month or two

My BP is a bit high anyway (I take enalapril and HCTZ) - if postural hypotension becomes an issue, I guess it would make sense to reduce one or both of these drugs

I'm fairly stoic about side effects, if Nardil gets this anxiety under control, I'll put up with them.

good luck jono, I'll be interested to hear how it turns out.

Is this under a psychiatrist or GP's care (especially curious given the MAOI+TCA thing)?

Maybe put the xanax on hold, and allow the phenelzine toget working, coz once you're up there when it kicks in you wont need a benzo. Definitely the best anti-phobia drug that can be prescribed. And it hit GABA receptors too, hence why its so good for social phobia. Once you're up on the phenelzine a benzo will be the last thing on your mind. Tho there's nothing wrong with 'as needed' in a tough situation.

Hi ggg, getting care from a psychiatrist here in Darwin unless you're psychotic and in the nut house is very difficult, none of the private shrinks are taking new patients. I amnaged to get a one off review with a psychiatrist, who suggested trying mirtazapine (which did nothing), after 8 weeks i went back to my GP with a heap of evidence about Nardil in panic and phobic anxiety.

The nortriptyline made him nervous, but i had evidence about that, specificialy that it was safe, and indeed, the presence of nortriptyline could actualy protect against a "cheese reaction"

I was determined to stay on nortrip as I have a strong endogenous component to my depression, and have always needed a drug that acted strongly on noradrenalin to control it

ah interesting jono .. psychiatrists are dime-a-dozen here, yet curiously i find sometimes it's GPs who are more willing to 'stray from the beaten path' in their approach. anyway, good luck .. hope you find success on Nardil!
