From marking down a volunteer in the trial as " improved" when the guy is actually being made worse from the med?

Lol yes.
Or what would keep the drug company from lieing by omission? Maybe not reporting certain data?

Obviously, it's well known that drug companies don't publish unfavorable reports, but in terms of counting someone as improved who is not, there are protocols. Generally things like the Beck Depression Index are used to measure results.

If the trial is 'double blind' the researchers nor the participants know who is actually receiving the medication and who is getting the placebo.

Do all drugs that get approved by the FDA require double blind studies?

This is why independent studies, conducted by a university medical school are much more highly regarded than drug companys inhouse "data on file"

Are you talking about deliberate and flagrant data falsification? I would hope that at the very least, fear of eventual uncovering would deter people. The one or two folks I've known in the pharmaceutical industry have been dedicated scientists who cared very much about doing their work with integrity. Of course there are bad apples in every barrel.
