When you know a specific combo works for you, puts you in full remission, but a pdoc simply wont prescribe.
Then if you do get it,insurance rejects it due to its high cost.

Yes, it can be very frustrating when you've had a particular treatment or combination of treatments work well for you only to have it become unavailable for one reason or another. That's sort of what happened with me with Nardil, only for me the medication essentially became unavailable because I received what was likely a bad batch of the stuff from my pharmacy and didn't want to take my chances with the medication any longer. I felt that my best option at the time was to try different alternatives, which is something that you may end up having to do. Is there anything that your doctor is willing to prescribe that's also covered by your insurance that you think might make for an effective add-on to your current medication regimen? I tend to think that if you're not doing well that it would be best to try something different, even if it's not the treatment option that would necessarily be your first choice.
