Last I heard, cariprazine was being submitted to the FDA and was supposed to be available later this year. That is wishful thinking, though. One of the articles I came across said that there were no clinical advantages to cariprazine when compared to Abilify. If anything, the side effects of cariprazine are more pronounced. It is more potent as a receptor ligand than is Abilify. Still, I can't help but to think that there will be at least some people who will respond better to cariprazine than to Abilify. That's just the way things are with psychotropics. We would need only one SSRI if everyone were to be equally responsive to all of them. Subtle differences in a molecule can make a huge difference clinically. If you add a single chlorine atom to imipramine, you get clomipramine. Clomipramine is considered to be the most efficacious drug for treating OCD. Imipramine is usually of little value in the treatment of OCD. If cariprazine were as safe to take as Abilify, I would welcome its addition to our armamentarium. As it is, I would expect that the startup side effects will be somewhat harsher with cariprazine than with Abilify. These include anxiety, insomnia, and a mild to moderate akathisia-like syndrome. It may be that there is also a greater risk of developing full akathisia. In addition, cariprazine can produce metabolic side effects and weight-gain. I was hoping to swap out Abilfy for cariprazine in order to lower my triglycerides. I was disappointed to learn that the substitution might not help to accomplish this.
