How should I titrate between Viibryd and Anafranil

Posted by Phidippus on October 21, 2011, at 14:06:12

I'm taking 200 mg of Clomipramine and starting on Viibryd. At what dose of Viibryd should I begin to taper off the Clomipramine?

This is something that you should consult with your doctor and/or a pharmacist (pharmacologist) on. While I don't think there should be any pharmacokinetic interactions, I'm not sure that a cross-taper is appropriate at all. If your doctor says to do a cross-taper, then by all means, follow his instructions.

Is that what you're doing? Clomipramine and its active metabolite both have long half-lives. If you want a shot-in-the-dark guess, I would say do some sort of cross taper changing the dose every 4-7 days. Do not follow my advice blindly.

Would I just stop the Clomipramine instead?

Yes. But again, what does your doctor say? If you are not receving these medications from a physician, I have no problem with that, and I'll do the best I can to help you. Honestly, though, you should consult with someone with a background in [psycho]pharmacology.

Thanks for your help, I appreciate it. My psychiatrist is prescribing the medications and has told me to cross titrate them, but gave no specific instructions on doing it. I'm taking Clomipramine 200 mg and I've got the starter pack for Viibryd which starts at 10 mg then titrates up to 40 mg.

Cross-tapering is probably the least bumpy ride of the two options mentioned above. Stopping clomipramine cold turkey can be nasty because of the so called 'cholinergic rebound' that you can get when quitting any anticholinergic drug. There's a range of side-effects, but basically you could feel like sh*t. This could provide enough motivation to taper the stuff down instead of tossing all of it away.

I'm not sure how Viibryd compares to clomipramine with regard to potency of serotonin reuptake inhibition. You'll probably need a high dose of Viibryd to even up with what you got on clomipramine. I understand that 40mg is the maximum dose.

Eric, how did/does dry mouth affect you as a side-effect of clomipramine? I find it one of the most annoying side-effects..

I don't get dry mouth from Clomipramine, however I got it so bad from Cogentin, I could barely swallow. I think I chewed gum to deal with it.

No dry mouth from clomipramine... Amazing!

I would clarify with him how exactly he wants you to do the switch.
