Jeroen not one of above. Sounds like you might want to wait for the Sapris did they change your diagnosis so sorry you are feeling so bad. Phillipa

Is it worth considering raising the dosage of Seroquel? I know that it's possible to go up to 800mg as a maximum daily dosage. Seroquel doesn't significantly block dopamine receptors until you hit 250mg or above.

jms600 that sounds like good advise, 150mg is low for antipsycotic treatment. isn't 300mg the teraputic dose for seraquil

Jereon, since you mentioned me, I'm responding. I'm not expert, just a chatty psych patient that is familiar w/ my own medicines.

I read your threads, but our afflictions are so different, I can't begin to make a reccommendation. My knowledge is not theory based at all.

I do know my heart is going out to you. Let other brains weigh in. Did someone mention saphris? My pdoc mentions that from time to time, but I have never tried it. He says it's the gold standard when one is in a particular crisis.

My prayers, of whatever stripe, or is it more acceptable to say thoughts, are with you.

Don't you ever read *my* posts? First of all, there are too many people on Psycho-Babble to keep track of, especially for those individuals, like me, who have memory impairments as a feature of their illness. Second of all, I have tried to impress upon you that Abilify can prevent other antipsychotics from working. I would consider swapping out the Abilify for either Risperdal or Invega and keep your dosage of Seroquel the same.

To prove you read this post, I would like you to comment on the idea of discontinuing Abilify. Risperdal should make an ideal substitute. Why? Because you have responded to it partially in the past. See, I remember. Furthermore, I have personally witnessed this principle operate with a female friend of mine.

Abilify 30mg + Seroquel 800mg = Inadequate treatment of schizoaffective order.

Risperdal 3mg + Seroquel 100mg = excellent response to antipsychotic and antidepressant properties of these APs.

Oh, yeah. I forgot. I had this conversation with one of my doctors. He agreed with me that a theoretical basis for this phenomenon is that Abilify is a D2 and D3 partial agonist. It acts to block or buffer the D2 antagonist activity of other antipsychotics. He has seen this happen in his practice. However, as one would expect, this is not a consistent finding. Another consideration is that the D2 binding affinity for Abilify is much higher than that of Seroquel. Seroquel is outcompeted by Abilify for receptor sites.

At what dosage?

Is there any chance you could start at a lower dosage and cross-titrate with the Abilify. I was sedated on Risperdal for the first week. The sedation disappeared. However, this was at a dosage of 1.5mg.

Navane would make a perfect adjunct to Seroquel if no other antipsychotics are acceptable.

maybe an anticonvulsant?

Have you tried anticonvulsants+antipsychotics? I *hated* depakote plus Abilify (the combo set off EPS whereas Abilify alone doesn't cause EPS for me), but it really works for some people. Risperdal+Depakote was, weirdly enough, easier to tolerate than Abilify+depakote. Just keep your Risperdal at or below 4mgs (I think that's the point at which Risperdal might start causing EPS).

I'm so sorry, I didn't realize that. I'm so sorry that your feeling so bad. The way your feeling right now will pass, I'm not sugesting your will magically have joy and happiness but this hopeless feeling your having right now will pass. We all have ups and downs and the ups don't last forever and the downs don't last forever either.

Are you psychotic on your current med combo? If not maybe you should stick to it. If you are still depressed maybe you should add an AD. If not you could try the new med Latuda(lurasidone) or perphenazine but that will just make you tired.

Yes nothing else. Except maybe for a movement disorder, which I treat with cogentin, and is not a problem.
