I have pretty bad obsessional OCD (no rituals, intensely bad thoughts) that result in substantial psychic discomfort and difficulty focusing on anything. Adderall helps with both. Unfortunately I am also bipolar (i take lithium and other mood stabilizers, an AP, but no AD) and it potentially can destabilize my moods (its not clear that it does or doesnt but pdoc thinks its too risky). Does anyone have any thoughts as to an alternative to adderall (maybe provigil?) that might help in a similar way? Also, is it weird that adderall helps with my ocd? I have read that it typically makes it worse. Thanks for the thoughts.

Not that weird - a recent study found that Dexamphetamine reduced OCD symptoms in refractory patients.
I'm not sure that any other stimulants are helpful for OCD; lots of accounts I've read suggest that Provigil will exacerbate OCD; same with Ritalin.

I don't know, but provigil would make my mind think and think intense weird thoughts even though I was exhausted. Very unpleasant. Somehow, I can imagine it intensifying OCD. It's not at all like dex for me. --2¢.
If you haven't cycled on adderall, and it helps.... There is at least one BPll sufferer that uses a little adderall because s/he's usually one the depressed end.... I hear you that it's a risk. Possible to do a small amount under doc's supervision?

It's true: methylphenidate (Ritalin) is useless, where as d-amphetamine (Dexedrine) or d-methamphetamine (Desoxyn) are effective.

I have found adderall brings on my OCD but that is def partially bc I never stick to normal dose, maybe I should not be an idiot and try a low dose of my script.
