Surprised at what I can eat on Parnate

Posted by Maxime on January 1, 2010, at 13:55:56

Hi

When I was on Parnate in 2002 I tried some of the forbidden foods. I am doing the same this time. I've been able to eat cheese (cheddar and swiss) and drink red wine without a reaction. I guess I am playing Russian roulette because I know that I could have a hypertensive crisis from these foods. In 2002 I had a crisis from the following: homemade red win (store bought was fine), soy sauce, cold remedy (I took this by accident forgetting that I was on the Parnate). I think I have been really lucky. Maybe my body doesn't produce tyramine. Who knows, but I am surprised with what I have eaten and how I have not reacted from it.

I continue to feel great on my Parnate and Nortriptyline! I am more social and less anxious as well. I think this med was made for me. :)

I read an article that being on amitriptyline interfered with the pressor reaction of tyramine so that those on amitriptyline were less likely to have tyramine reactions. I assume it would be the same for nortriptyline, so my theory is that it's because you're on nortriptyline that you can get away with eating some cheeses, etc. The rest of us would be in trouble!

Here's the article just in case anyone was interested:
Lancet. 1982 Jul 24;2(8291):183-6.
Will amitriptyline prevent the "cheese" reaction of monoamine-oxidase inhibitors?
Pare CM, Kline N, Hallstrom C, Cooper TB.
Administration of amitriptyline greatly diminished the pressor response to intravenous tyramine in patients receiving monoamine-oxidase inhibitors (MAOIs). Dothiepin and trimipramine, however, produced little change in sensitivity to tyramine. It is suggested that a combination of amitriptyline and an MAOI, started together in a modest dose that is then increased, may protect patients against the potential dangers of eating tyramine-containing foods. However, because MAOIs allow a high proportion of ingested tyramine to be absorbed into the systemic circulation, patients treated with MAOIs, even in combination with amitriptyline, should not be encouraged to eat foods containing tyramine.

That's really interesting. Thank you for sharing that. It does explain a lot.

That's one of the things my pdoc was talking about when he said why he likes to combine CERTAIN tricyclics with Parnate. He thinks I'm safer. But it can't be any old triciclic. I can eat a lot more things than I thought at first.

That's great you found that Peanut. Have you read the stuff from Dr. Gilmore in Australia...psychotropical.com?

according to Dr David Burns 'Feeling Good' book, page 582 (excellent read if you haven't already) it's possible to have pizza 6 times and be fine and on the 7th go into hypertensive crisis. Hence you might want to be a little careful! It also says that the amount of tyramine required for someone to go into crisis varies widely and it's unknown why this is so.

I use soy sauce and (perhaps stupidly) have drank some draft lagers but I'm too chicken to try something strictly off-limits e.g. strong cheese!

I'm just curious... what is the drug you can carry with you just in case you go into hypertensive crisis and how many of you carry it or have used it?

http://www.dr-bob.org/tips/split/Nifedipine-for-emergencies.html

I don't carry it but perhaps should. I do have a MedicAlert necklace though.

I have thorazine/largactil to take if I have crisis. Thing is, it doesn't work very fast.
