Feeling Miserable AGAIN!!!!!!

Ok- so now the feelings of anxiety and misery are back.... Pdoc on vacation..... What next? I have gabapentin 200 mg 3x/day and adderall XR 10 mg 1x/day.... Options I could try with current scripts include increasing gabapentin to 300 mg 3x/day or adding a second dose of adderall (or both). BTW: both are ok according to my current scripts.... anyone? Also, I meet with the pdoc in a few weeks and wonder what next step should be? Add an antidepressant???? If so, which one.... I keep asking and posting about stimulants because I am trying to figure out what is missing...... I think the gabapentin helps GABA stay afloat and I think the stimulant helps the dopamine stay present.... Is this right? If so then I would assume the missing links are NE and serotonin.... Thereofore, would parnate or pristiq be good choices???? Anyone?
I realize there are a lot of questions here so please respond to whatever makes sense..... I am desparate again.

And you've had a complete medical workup? Doesn't make sense to me unless the new med waa a placebo effect? What do you feel might help med wise? Phillipa

Hi FM,

I'm sorry to hear this. I don't know what to advise. Try increasing gabapentin for anxiety control? Not going from 600 a day to 900 but maybe increase one dose and seeing, if timed right, if that brings relief, and going from there. I hesitate to advise about adderall because (besides lack of experience) but also, when I increased my provigil one day (within my script) it fueled anxiety and dysphoric thinking. Granted, these are very different drugs.... And maybe you want more focus?

I spun out when my pdoc went on vaction for two weeks (just a few days ago) and I was feeling unstable to begin with. Just wondering if it is a trigger for you, too?

Does your doctor have a replacement whilst he is away? I would try to get an appointment with the replacement if possible. Don't mess with the meds yourself because it can be dangerous.

I hope you feel better soon.

I have taken up to 2700mgs of gabapentin a day so 900mgs is not a big dose and worth a try. What ADs have you tried?
Sorry you're feely lousy again. The ups and downs can be so discouraging.
Hang in there. And fill in the info about what you have tried. It will give people something to work with.

Still miserable and no changes.... Not sure what to do.... Only choices for right now are to up the gabapentin or up the adderall,... Not ready to up the adderall as I am irritable...nor ready to up the gabapentin as I am lazy.... I just think i need an AD right now- not sure why I dont have one on board..... I guess cause recent try with buspar and lexapro failed.... This sucks!

It would probably be in your best interests to be as objective as possible and understand that things might continue to suck for a few more weeks. It takes incredible patience and tolerance for pain to be able to give each treatment enough time to work.

I think I mentioned in another post that I am not a big fan of Neurontin. It doesn't seem to help that many people with depression or bipolar disorder, even though it is brought up to very high dosages. It just doesn't seem to "stick" with some people, despite producing an initial improvement. They just push the dosage higher and higher. Still, it might be logical to go up to 1200mg to give Neurontin a full trial.

Lamictal might be worth having on board as an augmenter as you work your way through other treatments.

I know Wellbutrin can produce irritability in some people, but it can also act to reduce depression and anxiety if it is a good match for your psychobiology. My doctor sees Wellbutrin + Lamictal to be particularly effective. However, I never quizzed him as to whether or not there were commonalities in the case profiles of those people whom profit from that treatment.

Thanks Scott- I am thinking of upping the dose of gabapentin tomorrow so I can try and be an active part of the celebrations instead of a couch potato.... :( I defnitely see how people need to up the dose though- one week at 600 was all i got.... I wont up this too many times- hope the 900 will do it for me... if not then next step will be to rethink this whole ordeal which really sucks.... I think you mentioned trileptal before.... Lamictal is out for me due to side effects (was on it before).....

Well, still not too great- having a hard time.... gabapentin is short acting and seems to need frequent dose increases....also isn't helping with the depression side of things..... Wish I could say I was better! I hate this! How are you?
