i mean, at what point does one check themselves into the hospital. i've been plenty suicidal at times, in incredible pain, but just figure that going into the hospital would make me feel worse, and label me as a 'mental patient' for the rest of my life.
but at what point.....shouldn't there be guidelines or something?

Uncouth when you no longer feel you can cope on your own. I know you've been feeling suicidal and helpless. I'd personally suggest now as seriously I feel someone needs to help you sort out your meds. Please go and keep us posted? Love Phillipa

Uncouth, I guess a lot of us have that fear of, when have we crossed the line. This is just my opinion, but I think it's when you are in a dangerous, painful place and have done all that you can do and you are still loosing ground. I know your concern about the stigma of being hospitalized, but if there is one thing we should have learned (finally), this is a real disease and people are hospitalized with real diseases everyday. Thoughts & prayers are with you.

Wondering if I will ever get my intelligence, energy and life back. be able to work. Be able to think and be smart again. What's kept me from the hospital is that I honestly think they are unable to help me and it would be akward to go in there saying I don't want live anymore. I've never been one asking for help.

Dear Uncouth. I'm sorry to hear you are feeling so awful.
I think you should go to a hospital or ER if you feel you are a danger to yourself or others. I think you have to be realistic about what a hospitalization can do for you. It's not therapy. They will focus on your meds and, if your lucky, may have some new ideas. It's certainly a safe place to try out new things. But if you think your pdoc is up to speed, it might make more sense to have a psychpharm consult. That'a another way to get a new set of eyes on what you are struggling with, what you are taking and what you have taken in the past.
That being said, if you do not feel safe, and can't take care of yourself, it might make sense to consider a hospitalization. Did you read the Daphne Merkin article that was posted here a few weeks ago? It's about her hospitalization for depression and I think gives a really good picture of the experiences both of depression and of the hospitalization. It was in the New York Times if you have trouble finding it here.
Try to find out as much as you can about the hospital you are considering. Who staffs the units? What is a day on the unit like? What is there approach or orientation? Hospitals vary a lot. There are few good ones.
Still, if you feel you can not make it on your own, I wouldn't let the stigma stop you. You have to take care of yourself and if a hospitalization looks like a useful step, you should take it.
I hope you feel better soon and please let us know how it's going. We'll all be thinking of you.Zana

The best place to ask this question is in the emergency room of your nearest hospital. They can help you figure out the options better than you can.
