New meds combo.. Advice?

I've been really agitated while really exhausted at the same time, so much so that I have to take a nap every day. Part of the tiredeness is my health, I found out (it's not all depression, yea!)and I'm working on that.

Here's the combo I've been on for the last year or so, when I finally convinced my pdoc to add Provigil:

60 mg Cymbalta
100 mg Provigil
.5 Klonopin up to 3X per day/as needed
Trazadone for sleep

So my pdoc added
25 mg Lyrica twice a day

because I had to take more Klonopin on some days to ease the agitation, which had grown to jumping out of my skin type of agitation in the last several months. (I know the Klonopin increases fatigue!).

I thought the logical thing would be to cut back on the Provigil, but that actually made things worse, so I went back on it (this was before the Lyrica was added)

So, any thoughts on this combo? My pdoc says this should reduce my use of Klonopin.

Cymbalta and Provigil may be causing your agitation. If you're treated for depression and have symptoms of agitation you may suffering from mixed state (agitated depression), which is usually treated with anticonvulsives/ssri and lithium.

Also lyrica can be sedating at that dose so maybe he's hoping that it will decrease the agitation without upping the benzo. Phillipa
