On some of the other boards, people say that Seroquel, Zyprexa, etc., works against Emsam. Can anyone explain the theory in laymen's terms? Thanks, Michael

back when MAOIs were more commonly prescribed, it was pretty common for shrinks to give the patient an old-school antipsychotic w/ the med, and in some countries there were combo drugs that had a little bit of some antipsychotic (I think phenothiazine-types) and the active antidepressant. Since MAOIs can be stimulating, short-term neuroleptic use could overcome that. Also, for a while, combined neuroleptic/AD drugs for depressive disorders was en vogue.
So..I don't know that you're necessarily "working against" the med, but it is a little weird. It seems like one med is marinating your brain in neurotransmitters, and the other is effectively blocking parts of your brain from using the high levels of neurotransmitters..its kind of like the amphetamine+neuroleptic combo, which apparently works for some people.
I'm not a big fan of neuroleptics, so I'd say ditch the zero and just try the EMSAM, but I don't know your situation. I think if you agitation/anxiety is a problem, you'd be better off either using a benzo or trying another med (not neuroleptics), but again: that's personal preference, colored by my intense dislike of neuroleptics.

Yes, there was a drug combo here in the UK called Parstelin (Parnate and Stelazine). My old psychiatric nurse said it was very effective in mixed depressive and anxiety states.

I believe the complicated interplay between ensam and antipsychotics makes it fruitless to try to predict the end result. The people on the other boards were probably looking at it in an overly simplistic fashion. That is, ensam is going to increase dopamine but seroquel is going to block it. It just isn't that simple. Seroquel is actually increasing dopamine too, as well as blocking it, but it is doing all of this in varying degrees in different parts of the brain. And then when you introduce the interplay with serotonin and histimine, it gets totally out of the realm of prediction. And that's not to mention their unknown and complicated actionis on hormones and genes and other fine details that aren't usually discussed, many of them unknown. That's the way I see it anyway.
Ensam+seroquel might be awesome for one person and useless for another. Just saying one is going to counteract the other demonstrates a lack of respect for how complicated these drugs are.
