akathisia vs hypomania/agitation

Is there some way to tell if a condition is akathisia versus it being some kind of agitation or mixed hypomania? Here's my situation....

I went off of EMSAM about 2.5 weeks ago after having been on it for nearly 6 weeks. After about 2 days off it, I started to get really "revved up" as my psychiatrist calls it. I just felt very agitated, both physically and mentally. Sometimes I would have to pace and I felt like my mind was spinning. I've been in these states before, sometimes due to taking certain drugs, and other times not caused by drugs.

Anyways, it's been like this for the past 2 weeks or so. I went back on EMSAM four days ago because I wasn't sure that I had given it a fair trial. THe condition doesn't seem any better or worse now that I'm using EMSAM again. In fact, I'm not even sure that it's related to the EMSAM at all.

It's very physical, as in I feel like I need to fidget a lot and jiggle my legs, etc. But I do sort of feel just an overall sense of revved-up-ness, physical and mental, as if I drank too much coffee or something. It's not as negative as it has usually been in the past, when I wanted to kick and punch and throw things.

Also, it seems to get worse as the day goes on. My theory about that is that I take Seroquel at night, and I think that later in the day my levels of Seroquel are much lower so these agitated symptoms are more apparant.

Any thoughts?

Yeah, I've had this happen to me several times, usually when a dose goes up or down or when stopping a med. I don't know what to call it. It does fit descriptions of akathisia. It also fits descriptions of certain types of mania.

I think these kinds of things are more prone to happen when an antipsychotic is involved. That is just pure opinion though. I never had those kinds of problems until after being on low dose zyprexa for a few years.

Benzos kind of dampen it down but not completely.

For me depakote 500mg extended release nailed it in about 24 hours.

what does your pdoc think it is? Love Phillipa ps I'd love to see you succeed on the patch.
