Hi all. I posted here a while back about medication for intractable OCD. In the interim, I was given a presumptive diagnosis of comorbid Bipolar II. My current doctor recently started me on a cocktail of Prozac (40mg), Lamictal (50mg), and Zyprexa (2.5). He now wants to raise the dose of Zyprexa to 5mg to help me deal with contamination obsessions that, frankly, border on delusional. I initially opposed the addition of the Zyprexa in the first place due to concern over the possible metabolic effects. Specifically, I was concerned that the Zyprexa would have a ruinous effect on my physique, or worse, would cause me to become insulin resistant. In reality, I really have no context for these fears: How long does it take for "metabolic syndrome" to develop? Does everyone develop it? Are there any outward signs of developing it? Is any damage done irreversible? I should also mention that my maternal grandfather suffers from diabetes type II. Given my concerns and family history, would it not make more sense to try one of the other APs less likely to cause metabolic problems, e.g. Risperdal, Geodon, etc.? I think he is partial to the Zyprexa because it has mood-stabilizing properties, and according to him is "best for quasi-delusional thoughts," but are the other APs really any less effective? For what it is worth, I do not think he intends to keep me on the Zyprexa long-term, and in fairness, I have not noticed any weight changes or change in appetite in the two weeks I have been on the Zyprexa, but I am still concerned over any possible short term effects. I would like to hear from any one with comments or suggestions. Thanks in advance.

I don't know if this will help but when I was on Zyprexa I immediately noticed the increase in appetite - the very next day. So I'm guessing that if you aren't experiencing it yet, you may not at all. I was taking 2.5mg also.
As far as the metabolic syndrome, I think it takes some time to develop, months anyway. I don't think it would be much of an issue short term and at a low dose.

Hi Marsha. Thanks for the reply! I'll take it as a good sign that I haven't noticed any change in appetite or weight in the two weeks that I've been on Zyprexa. It's so hard to become expert in an entire area overnight. I generally like to research before acceding to a doctor's order (much to their chagrin), but I'm absolutely clueless when it comes to APs.

5 mg is not terribly high but still enough of a
dose to cause appetite increase. I've heard the
sublingual Zyprexa has less of an effect on appetite.
There are also lots of bug a boos about
diabetes. My son's blood sugar went up after starting
Zyp. but his Pdoc warned us to cut down
on fast foods and sweets. Now he is a little
overweight but blood sugar is normal.

I started on 5mg and it was crazy. Within days I was eating gobs of food and I was having cravings for certain things that in the past I didn't like.
Where Seroquel had brought my weight down from 115 to 105, Zyprexa brought it back up to almost 120 in less than two weeks.
