OK...in my experience, ADD/ADHD is really best treated with: stimulants, provigil, straterra (in that order). What might work is *very* low-dose trileptal plus a stimulant...sometimes you can lower the dose of the stimulant (so weight loss won't be so much of an issue) and you get better mood control that way. Thing is...mood-stabilizers can help concentration, but only to a point; after a while, they actually start to make people stupid. Sometimes very, very stupid.

the doc wants to put him on this for a month to control his mood then he will put him back on the concerta. he will possibly raise the concerta again. the teacher said he is still having problems with hyperactivity at 36mg; he said the meds is not working. so is there any advice i can tell the psyc maybe up the concerta and get him off the trileptal i really dont like this med.

ADHD can be kind of tricky. You could try focusing just on stimulants to control it, in which case you'd probably want to up the Ritalin and see if just ritalin (concerta, ritalin LA, focalin, etc.) would be sufficient to keep everything on a more or less even keel. You could also try a novel approach--like using Provigil, or ritalin plus dexedrine, or something like that. Trileptal *could* help with mood swings, but having taken it as an adult, I kind of feel like it could be overkill. Also, maybe you should ask the teacher to be more specific. If your child's grades go up on medication that should be the primary indicator, not the teacher's report. In my experience, some teachers really expect meds to do their work *for* them--they feel like any annoying kid needs to be medicated. Its not fair to parents, and its certainly not fair to parents. Objective measures of performance--standardized test scores, grades--are important, as are other factors, like social functioning.
Also, some non-drug approaches to ADHD have proven very helpful--for example, bio-feedback helps some, as do forms of "awareness training" and similar therapeutic techniques. Depending on the severity of the condition, this can be done alone or in conjunction with ongoing medication-based treatment.
