I'm on enough opiates to slaughter a stable of horses.....I'm trying to taper dowen with xanax...but isnt wokring very well ....I'm on 120 oxycodone IR 30mg's for brealthrough pain, 30 oxycontin 40's,and 360 methadone tens.....I was gonna have my doctor lower me,but I cant because I really am in horrible pain and I need the omeny.....SMYWAY,does aanyone know any potentiators besides grapefruit? And how does that work exactly? as far as if you eat the fruit,then how long do you wait before dosing?...Sorry for the semi-incoherent post.....i'm very VERY stressed right now...someone get back to me please.. You won't be able to taper unless your pain is under control. What type of pain do you suffer from? It sounds like you'll need to explore some novel non-opioid treatments.

Do you mean that you're on 3.6g methadone a day, (amongst the rest)? I'm wondering if I read your post right. What a worry. If that's right you can't feel well, quite apart from the pain. I tend to think that any increase in available opioid to your system will simply be swallowed up by tolerance.

I'm very sorry your haveing a rough time. You must be in a lot of pain!
The only thing that comes to my mind is DLPA (DL-Phenylalanine). You can get it from healthfood shops and it helps by prolonging the action of your body's own 'pain killing hormones'. I dont know what effect, if any, it would have on the opoid medicines you're taking, but maybe it might help you whilst tapering down. I think it can take a few weeks to start working. A google search will give you many results. Here's one link to try:

Seems like two problems.
First, the opioids. More and more is not going to help and will only deepen the troubles. Sounds like a professional detox center might be needed. They can put you to sleep for 3 days to get through the worst of the withdrawals and monitor you closely. They know what they're doing.
Second, the pain. I don't know what the options are. But there must be other options besides opioids, which just keep building tolerance every step up. I think either a good doctor or a change of doctors is needed.
Sorry things are so bad! Most of us here are not in good shape either, so you aren't alone.

Based on what I've read, you will want to use memantine (Namenda, Axura) to prevent further tolerance buildup, as well as adding proglumide (Promid, Milid) every other week to start the process of reversing the tolerance.
Depending on the severity of your pain, you may want to consider buprenorphine (Temgesic) for moderate pain, or fentanyl (Duragesic) for heavy pain.
It is also possible to obtain fentanyl "candy" (Actiq), which you roll around in your mouth for rapid effect, to treat breakthrough pain (typically in cancer treatment).
How severe is your pain, what is its cause, and what is your goal?
