Nardil - Do I really need to wait 2 weeks?

Hi,

Do I really need to wait two weeks after stopping Nardil and going onto 60mg of Seroxat? If so why?

I'm desparate to start something soon.

Yes. You really do have to wait until the Nardil washes out of your system. You want the monoamine oxydase to build back up in your system. You also don't want to risk elevated SE levels that might lead to Serotonin Syndrome.

I switched from isocarboxazid (marplan) back to Nardil recently. I had a 10 day gap and when I restarted the Nardil I had what I can only describe as 'excess Serotonin' for about 3 days. Had I started sooner I think I may have had full blown Serotonin Syndrome, or worse.

Though from what I've read on here, Parnate seems more tolerable combined with other MAOIs.

As for SSRIs, my doc told me he added Sertraline to one of his patients Nardil regiem, and that it "effected him badly", despite there being text and studies on this combo. Luckily his patient was wise enough to stop in time.

Yes, unfortunately.

Nardil works by permanently shutting-down the factories (MAO enzyme) that are used to break apart excess serotonin in the neuron (nerve cell). The problem is permanence - the factories are essentially destroyed. Even after the last of the Nardil has disappeared from the body, the neurons must still rebuild a supply of factories. This rebuilding process can take two weeks. In the meantime, levels of serotonin begin to build up. When a SSRI or SRI drug is introduced before the factories have been rebuilt and operational, there is a high risk of a dangerous excess of serotonin that accumulates in the synapse (gap between neurons) that can result in a sometimes fatal reaction called serotonin syndrome.

I'm all for sane washout periods, but in this case unfortunately the washout period is necessary. Nardil is an irreversible MAO, and it takes around two weeks for de novo protein synthesis to make enough MAO to allow you to safely take an SSRI.
