Please, i'm in severe chronic pain and was taking 60mg of SR oxycontin three times a day (180mg per day) plus about 15mg breakthrough oxycodone each day.
What would the equivilant Kadian SL morphine be?
my dr gave 100mg Kadian twice a day (200mg a day) and 3-4 15mg breakthrough IR morphine.At first it didn't work at all but after a few days it's working but WAY more sedating than the oxycontin was.
i just want to find out what the exact equivilant is. The medicines work for pain but are so different. The Kadian is so non-dependant on the exact hour that you take it, in contrast- the oxycontin would start wearing off at exactly seven hours later so i was never late for a dose- without much suffering. The Kadian doesn't stop working so suddenly but has the bad effect of sedating so i hate to take it.Is 180mg oxycontin equal to 200mg daily dose of Kadian (slow release morphine)?

There is no exact equivalent. As you say, they are quite different.

Perhaps you could try a different opioid. If your doc won't prescribe OxyContin, Duragesic might be a suitable alternative to morphine.

Thanks Ed, for responding. i was hoping someone would.
i tried Duragesic patches last year but became so sick because i work in greenhouses where the temperature is usually above 115 degrees F in the summer and i'm running around so much that it releases too quickly. i need a high dose because of the severe pain and just weigh 100 lbs so OD is easy.
so discouraged.Dr won't prescribe anything else and wants to take me off of opiods anyways.
Had a horrific time getting a nerve block procedure.Tears. The source of my pain is not apparant and he's backing out on treating me at all. which makes everything impossible - i can't live in the severe pain. Hopefully the nerve block steroid injection will begin to help in 3-7 days as he said and will help. It was a nightmare to get today- my Dr is sadistic - he wouldn't give sedation as planned beforehand. i never want to trust him again but have no one else to treat me.

Will he allow you to keep taking morphine? Caffeine might reduce the drowsiness.

Maybe he's scared that you might overdose. Perhaps he'd allow you to keep taking opioids if you got more frequent prescriptions eg. once a week from your GP.

If you can, you should stop working in GREENHOUSES! You run the risk of being re-exposed to fungicides at your work, which is most dangerous to you. Also, have any of your doctors tried you on paxil? it is sometimes used for fibromyalgia, which is very painful, and it can work in conjunction with other meds.

Oxycontin is slow release oxycodone right?
Milligram for milligram oxycodone is stronger than morphine when taken orally but has a shorter half-life. The slow release factor will complicate calculation, as will the poor oral absorption of morphine, but maybe oxycodone 1.5- 2mg = morphine 3mg. And of course morphine is more constipating than oxycodone, especially when taken orally. I find oxycodone more fuzzy or something.

Morphine, any day of the week. Although it is constipating, but less so when given by injection.

Interestingly, one study suggested that oral morphine was less constipating than oral oxycodone........ not in your case though!

cacti and succulents are so beautiful some times of the year.
i work for a city and grow plants for the municipal gardens and other city office buildings.as much as i hate to admit, i think that the steroid injection i had on Mon. is working. When 12 hrs was up last night, i wasn't in terrible pain as usual. Still i took it a few hours later and feel even groggier and this is the first morning in a long time that i didn't need both medicine and breakthough medicine in order to take first thing and then wait 40 min to get out of bed.
The procedure was so difficult and triggering to have. i don't know if i could have another one.
Maybe a different dr.This dr is so negative- he's bothered that i have a psych dx and no certain dx. He doesn't believe me or want to treat me. Their office person relayed direct lies about me to my psychiatrist- saying that i 'took too much medicine.' maybe it was an ambiguious statement but i was never off one single dose. i always took eaxctly what was prescribed and always came out right. There's always such a negativity at that office against me.
i keep asking the four gyn drs that i saw this year for pelvic pain to send test results and visit notes and they say they haven't received them yet. This has been happening since Feb. It's so frustrating.i don't know what to do now. the psych problems are impossible - at least the pain is letting up. i'm nervous about going down on medicine but maybe the injection will be enough for a while and i should do it.
i wish i had a good dr that didn't make me feel worse because i have PTSD and DID- he probably knows- why else would he treat me so badly, like i was always lying about something? As if it wasn't hard enough to live anyways and impossible to live with this problem.Thanks for being there,

There are separate parts- i have DID and am not able to communicate with parts. you're right. it would be better for my health if i didn't work there. The work part isn't careful either so many time- not wearing gloves etc, but at least they took me off pesticide applications since the bone marrow problem came up. i'm so glad they took me off because i would do it still when i'm there. When i'm there everything is so different -i only remember a few things of what happens there- enough to make me worry but i can't change it. There's no one in charge or 'host' part that can do that.Paxil gave me a terribly bad migraine when i tried it and made switching so much more physically severe.
Thanks for responding and trying to help. it's so frustaring for me to have this problem. i worry about it and hate going to work until i get there and then when i do it's my entire life. That part hates me because i'm always so undependable and late- even now- writing here when i should leave. It's a balance i have to keep though because we're falling apart,

Yes, oxycontin is the SR form of oxycodone- i remember the dr saying that.
What you said is exactly right- how it feels when you take it. Morphine lasts longer.
i don't think the 200mg morphine and 180mg oxycontin was equivilent. Generally the oxycontin worked better to take away pain.Oxycontin worked so much better- i literally had no pain once if i was careful about the time but you're right about wearing off. i was in so much pain though every morning that i had to have it at my bedside to take with breakthrough first thing and wait in bed until it worked. It also had a bad side effect of making the muscles in my face and eyes feel tense or something like that- can't explain. Maybe because of the high dose- it was 60mg three times a day. Morphine doesn't have that effect- it works gentler but kept me in some degree of pain.
Nothing is so bad as being in terrible pain, though. Anything is worth getting out of pain if it's bad- i don't care about side effects.
