Depression, OCD and psychosis

Hey all. I'm feeling pretty depressed at the moment :( and hearing voices, very paranoid and my OCD is annoying me but my OCD isnt as bad as it usually is. I dont know what to do. I'm on a lot of medication but still feel depressed. Im starting to think my Mother and my Boyfriend are trying to pooson me. It feels real when it happens but then I know its not real later if that makes sence.

I'm on 10mg of zyprexa (which is doing nothing but making me fat) 200mg of clomipramine, 10mg of diazapam and 1000 mg of valproate.

What could I have added for depression? Should I increase the clomipramine? My pdoc was talking about going upto 250mg but im kind of scared of doing this.

He is talking about puting me on an older neuroleptic. Largactil(Thorazine) was mentioned but there is no way id ever go back on that stuff again.

He thinks I have a psychotic disordre :( I dont want to have this label. I hate labels. I wonder which one he is leaning towards.

Any help or info would be greatly appreciated.

You mentioned that your pdoc thought you had psychotic disorder and you were uncomfortable about that. I just wanted to say that I have known several people who seemed to be psychotic, but then after their depression and anxiety were brought under control (through the combo of antidepressants and antipsychotics) they were back to themselves. Did they really have psychotic disorders, or where they experiencing other mental health probs? It is so hard to say. The waters are so murky with diagnosis. Sometimes it is hard to say what is going on until after it clears up. So, don't be too concerned about what it is all called. No one really knows anyway.

Also, have you tried Abilify instead of Zyprexa? It is newer on the market, but I have heard some positive things about it.

Also, even though you eventually realize that your paranoid thoughts are not real, it is impt to tell your family and/or pdoc about them (which is probably only possible after you realize that they aren't true). It will help figure out how well (or how poorly) your meds are working.

There's some evidence that Seroquel can be useful for bipolar depression.

Thanks. I have tried Abilify. I have tried all the atypicals available in Australia, plus the very first anti-psychotic invented Largactil(Thorazine) and Id never want to go back on that drug.

I really dont know what to try next. I hope my doctor works something out.

The difference between a psychotic person and a mentally well person who is paranoid is that the paranoia is logically justified in a mentally well person. If the logic adds up that your boyfriend are poisoning you (unlikely, but possible, with the messed up ppl in the world), then it might be true. However, if you can't logically justify your feelings, then that's paranoia within the psychotic range, and medications can reduce the illogical feelings.

It's normal to be uncomfortable with the "psychotic" label, but it's only that way because of society's views on it. In all actuality, you needn't worry about labels. Often they're wrong anyway. What you need to worry about is that you're hearing voices and are excessively paranoid. So what are your options?

First off, maybe the doctor misdiagnosed you. If you're actually bipolar, the clomipramine could be making you manic. Everyone's mania is different. Maybe you need Zyprexa and a mood stabilizer and no antidepressant. You could also have psychotic depression, which I'm sure how that's treated.

But how bad are the voices? Are they significantly interrupting your day? If so, maybe an older antipsychotic will help that. If not, then I'd def avoid the older ones. You seem to be pretty coherent with your post, so it doesn't seem like you're too impaired (not that that's a great way to judge people).

I'm no doctor, so I can't say what's best, but I'm just here to tell you that you prolly have a lot of options available. Right now might just be a bad combo for you. Hope you find something that works!

I think Ed's right about the seroquel. Not only is it used for psychosis but for anxiety, biplar disorder. And BTW are you under severe stress right now. This can turn a depression into a psychotic one. Fondly, Phillipa
